An Integrated Group of Forms for Automated Psychiatric Case Records

Abstract

Six forms for recording psychiatric status and history, suitable for use in an automated record-keeping system, are now being used in a collaborative effort involving seven states and the District of Columbia—the Multi-State Information System for Psychiatric Patients Project.
