Depression as a Risk Factor for Coronary Heart Disease Mortality

IN THIS issue, Penninx et al1 report the results of a longitudinal study of depression as a risk factor for cardiac mortality in elderly, community-dwelling residents of Amsterdam, the Netherlands. This study joins a growing number of methodologically sound investigations showing that depression increases the risk of cardiac mortality, yet some fundamental questions remain about depression as a cardiovascular risk factor. Their study addresses 2 such questions that have both mechanistic and clinical relevance.

The first is whether only individuals with major depression are at increased risk for mortality, or if the risk extends to patients with less severe depression. If only individuals with major depressive disorder are at increased risk, we should be searching for mechanisms that are present in major depression but not in milder depressive syndromes.
