We can understand that Mr Der and his colleagues are unhappy that the recently published results from our Promotion of Breastfeeding Intervention Trial (PROBIT)1 disagree with the results of their observational study.2 But in their attempt to undermine our study, they have substantially distorted both our methods and our findings.
As we discuss in the "Comment" section of our article,1(p582) the 7.5-point increase in verbal IQ observed in our experimental group was larger than that observed for performance IQ and larger than the verbal IQ increase observed in our blinded audit. We say quite clearly in that section that the smaller differences seen in the audit, the teachers' ratings, and our observational analyses of associations with breastfeeding exclusivity and duration "may provide a closer estimate of the true treatment effect."1(p582) Given the high intraclass clustering in the IQ measures, however, . . .
